I confirm that my email address and phone number is correct

I understand that a no-call no-show will be charged the FULL PRICE of the service

I understand that I will only have a GRACE PERIOD OF 15 MINUTES. This is to ensure that the order of appointments or commitments after mine will not be affected of my rescheduling/cancellation request

I understand that dependent on how my skin retains pigment in rare instances I may need more than one touch up not due to any fault of the artist

I understand I am not allowed to bring a company (family/friend) and I am the only person allowed to enter the studio for the service

I understand that payments are non refundable regardless of reasons and is non-transferable to a different type of service

I understand that if there are any changes in my schedule, I have to inform Facebeauty Bar atleast 24-48 hours before my appointment

I do not have a previous eyebrow tattoo visible as that is considered a correction and will be a different appointment type 

All information provided to Facebeauty Bar will be treated with utmost confidentiality. We assure you that your personal details will not be sold or shared with any third party

Your information will be used solely for business reference and to assist you in receiving the services we offer. We prioritize the security of your data and maintain strict privacy standards

In a case the artist can’t meet up for your appointment due to unforeseen circumstances, she has the right to give you a 48-72hrs notice along side an immediate reschedule, but If in a situation a reschedule is not ok by you, a full refund will be made 

I solemnly agree NOT to engage with Facebeauty Bar staffs on job offers elsewhere, as I am aware that the CCTV cameras are active and recording. If found guilty, Facebeauty Bar has all the rights to BAN/BLOCK my future bookings

I agree to be very nice to workers in the studio and in a case of tipping after good services , only CASH gift is permitted
